In the setting of a porcelain aorta, in patients presenting with aortic stenosis associated or not with severe coronary diseases, percutaneous procedures (Transcatheter aortic valve replacement: TAVR, Percutaneous coronary intervention : PCI) are an effective and minimally invasive solution for these high-risk surgical patients. However, the intraoperative discovery of a porcelain aorta or a heavily calcified aorta during planned surgical aortic valve replacement (SAVR) combined or not with coronary artery bypass grafting (CABG) is often stunning and discouraging. This condition often leads to an aborted sternotomy in the hope of performing transcatheter procedures, the feasibility of which may be hampered by anatomical considerations. Moreover, for economic reasons, the majority of these patients with aborted sternotomies will be disappointed and will unfortunately not be able to benefit from TAVR. In this setting, off-pump anaortic CABG combined with a clampless SAVR under hypothermic circulatory arrest (HCA) can safely overcome an unexpected porcelain aorta and avoid aborted sternotomies, despite relatively longer HCA than total ascending aorta replacement. Transapical TAVR or SAVR with sutureless valve reduce operative difficulties and improve postoperative outcomes. In this literature review, we have detailed the different techniques of arterial cannulation, as well as the different published surgical techniques for successfully performing a SAVR combined or not with a CABG in front of a porcelain aorta.
