Coronary artery bypass grafting (CABG) is the gold standard procedure for myocardial revascularization in cases of multiple coronary diseases. The long term outcomes of CABG heavily depends on graft patency. Early graft failure, which can occur during or shortly after CABG, remains a significant issue, with reported incidence rates ranging from 3% to 10%. Graft failure can lead to refractory angina, myocardial ischemia, arrhythmias, low cardiac output, and fatal heart failure, underscoring the importance of ensuring graft patency during and after surgery. Technical errors during performing anastomosis are among causes of early graft failure. To address this issue, various modalities and techniques have been developed to assess graft patency during and after myocardial revascularization surgery. These modalities aim to evaluate the quality and integrity of the graft, thereby preventing these complications. In this article, we aim to discuss the strengths and limitations of all available techniques and modalities, with the goal of identifying the best approach for assessing graft patency during and after myocardial revascularization surgery.
