Abstract

Tooth avulsion is an emergency where the prognosis depends on extra-oral time and the storage medium. This study reports the case of a 10-year-old boy who presented 24 hours after the avulsion of tooth 21, which was initially stored in dry conditions. Despite a delay far exceeding the IADT 2020 guidelines, replantation was attempted to preserve aesthetics and bone capital. The protocol included atraumatic replantation, flexible splinting, and antibiotic therapy. However, follow-up was complicated by patient absenteeism at control appointments, which favored the early onset of inflammatory resorption. Rigorous endodontic disinfection combined with MTA apexification successfully stabilized the tooth. The one-year follow-up shows a functional tooth and preserved bone capital. This case highlights the importance of patient cooperation in the success of late replantations.
