Abstract

Introduction : the immature platelet fraction (IPF) is mainly used in the differential diagnosis between peripheral and central thrombocytopenias. The objectives of our study is to determine the reference intervals of the immature platelet fraction in our population, to evaluate its ability to differentiate between central and peripheral thrombocytopenias, and to define a threshold that discriminates between the two mechanisms. Materials and methods : we worked on 127 samples, including 81 controls for determining reference intervals, and 46 thrombocytopenic patients. A platelet count and an immature platelet fraction measurement were performed using the complete blood count (CBC) MINDRAY BC6800 PLUS automated system. The comparative study was conducted using the Mann-Whitney test, and the discriminating threshold was defined using the ROC curve. Results : the average of the immature platelet fraction in the controls was 07%, our reference values ranged between (1.6% and 14.76%), 56.52% of the thrombocytopenias were of central origin. Our study showed a highly significant difference in the results of the immature platelet fraction between patients with peripheral thrombocytopenia and those with central thrombocytopenia. The discriminatory threshold for this fraction between the two etiologies of thrombocytopenia was 11.75%, with a sensitivity of 100% and a specificity of 95.7%. Conclusion : the use of the immature platelet fraction effectively guides the etiological diagnosis of thrombocytopenia.
