Introduction: Posterior reversible encephalopathy syndrome (PRES) is a rare neuroradiological entity characterized by reversible vasogenic edema, often associated with hypertension or systemic stress. Clinical Observation: We report the case of a 76-year-old hypertensive woman who underwent surgery for sigmoid adenocarcinoma. Forty-eight hours after the procedure, the patient developed generalized tonic-clonic seizures progressing to status epilepticus. Brain MRI revealed supratentorial and infratentorial vasogenic edema, predominantly in the frontoparietal regions. Mechanical ventilation, sedation, and antiepileptic treatment with phenobarbital led to progressive improvement, with extubation after 48 hours and complete recovery, except for a transient left-sided hemiparesis. Conclusion: Postoperative PRES remains rare but should be considered in any acute neurological manifestation following surgery, especially in hypertensive patients. Prompt diagnosis and appropriate management are key to a favorable prognosis.
