Abstract Objective: To investigate the link between inflammatory ultrasound signs of hand osteoarthritis (HOA) and clinical symptoms. Patients and methods: Our study was conducted on 116 patients meeting the American College of Rheumatology criteria and the modified Kellgren-Lawrence (KL) radiographic score for digital osteoarthritis. We examined 28 joints per patient and collected inflammatory ultrasound data and clinical data (painful and swollen joints, overall finger pain intensity and pain on palpation, stiffness, fatigue, and functional impact using the Dreiser score). We studied the relationship between the different clinical and ultrasound scores using Pearson's correlation coefficient. Results: Results: A total of 3248 joints were examined. Synovitis was strongly correlated with joint swelling and moderately correlated with joint pain and the Dreiser score. Effusion was weakly associated with all clinical scores (except fatigue). Power Doppler showed a significant but weak association, and only with clinical synovitis. Conclusion: Ultrasound-detected inflammatory lesions (synovitis, effusion) are associated with the clinical signs of digital osteoarthritis, particularly at the joint level.
