Chirurgie à Cœur Ouvert D’un Fibroélastome Papillaire Du Pilier Postérieur De La Valve Mitrale. à Propos D’une Observation Open-heart Surgery For A Papillary Fibroelastoma Of The Posterior Papillary Muscle Of The Mitral Valve : A Case Report

Résumé Le fibroélastome papillaire est une tumeur cardiaque bénigne très rare de localisation valvulaire préférentielle. Parfois asymptomatique, il peut être à l’origine d’accidents emboliques graves. Actuellement, le diagnostic est facilement évoqué en échocardiographie. L’exérèse chirurgicale de cette tumeur est un traitement sûr et efficace permettant de prévenir les risques de récidive embolique. Nous rapportons le cas d’un patient présentant une douleur thoracique angineuse. Les échocardiographies transthoracique et transœsophagienne ont mis en évidence une tumeur valvulaire mitrale. L’examen anatomopathologique de la pièce opératoire a confirmé le diagnostic de fibroélastome. L’exérèse simple de la tumeur a mis le patient à l’abri d’un accident embolique. Abstract Cardiac papillary fibroelastomas are very rare and benign tumors, usually involving heart valves. They are sometimes asymptomatic. However, they become serious illnesses when associated with clinical events, as systemic embolism. Diagnosis is evoked by echocardiography and confirmed by histopathological examination. The only curative treatment consists of the excision of the tumor under cardiopulmonary bypass. This approach is safe and prevents the risk of embolism recurrence. We report the case of a patient with anginal chest pain. Transthoracic and transesophageal echocardiograms revealed a mitral valve tumor. Pathological examination of the surgical specimen confirmed the diagnosis of fibroelastoma. Simple excision of the tumor protected the patient from an embolic accident.
