This study aimed to compare the effects of two rehabilitation programs—an aquatic program and a land‑based program—on functional mobility and quality of life in women with knee osteoporosis. An experimental design with two equivalent groups was employed. The sample consisted of 30 women aged 45–60 years, who were randomly assigned to either an aquatic training group or a land‑based training group. Functional outcomes were assessed using the Timed Up and Go Test (TUG), the Six‑Minute Walk Test (6MWT), range of motion (ROM), the Visual Analogue Scale (VAS) for pain, and the WOMAC Index for quality of life. The results revealed statistically significant improvements in both groups across all measured variables. However, the aquatic training group demonstrated greater improvements in functional mobility, endurance, range of motion, pain reduction, and overall quality of life. These superior outcomes are attributed to the physical properties of water, such as buoyancy and hydrostatic pressure, which reduce joint loading and allow for safer and more efficient movement. The study concludes that aquatic training is an effective and safe rehabilitation option for women with knee osteoporosis. It recommends integrating aquatic exercise into rehabilitation programs and combining aquatic and land‑based training to achieve optimal therapeutic outcomes.
