Spermatic cord torsion is a urological emergency whose prognosis depends on the promptness of diagnosis and management. It rarely occurs in a testis previously operated on for cryptorchidism, making the diagnosis more challenging. We report the case of a 15-year-and-5-month-old adolescent who had undergone left orchidopexy for cryptorchidism 10 years earlier and was admitted with acute left scrotal pain. In the absence of Doppler ultrasonography during the night shift, urgent surgical exploration with detorsion and orchidopexy was performed, resulting in a favorable outcome. This case highlights the importance of considering testicular torsion even in patients with a history of previous orchidopexy.
