Primary renal neuroendocrine tumors (NETs) represent less than 1% of all renal tumors and pose a significant diagnostic challenge as their radiological appearance often mimics conventional renal cell carcinoma (RCC). We report the case of a 52-year-old female who presented with isolated right-sided flank pain. Imaging revealed a 5 cm solid right renal mass, leading to a pre-operative diagnosis of RCC and a subsequent radical nephrectomy. Definitive diagnosis of a primary renal NET was established through histopathology and a comprehensive immunohistochemical (IHC) panel, which showed strong positivity for chromogranin A, synaptophysin, and CD56, while being negative for renal epithelial markers like PAX8. The metastatic workup was negative. Given the unpredictable nature of these tumors, the patient was placed on a long-term surveillance program. At three years of follow-up, she remains asymptomatic with no evidence of recurrence, representing an encouraging short-to-medium term outcome that nonetheless underscores the need for ongoing vigilance.
