Abstract Introduction: The absence of standardized procedures can compromise patient safety in surgical oncology. This study evaluates the impact of implementing organizational care procedures on the management pathway of patients with cancer and perioperative safety. Materials and Methods: A longitudinal "before-after" study (2017 vs. 2018) was conducted, including 334 patients operated on for confirmed digestive cancers at the Djillali Rahmouni Hospital (n=167 per period). The 2018 intervention included: the creation of a personalized patient pathway, reorganization of the operating theater (flow control, staffing), strict hygiene protocols (skin preparation, antibiotic prophylaxis), and proactive pain management. Results: Both cohorts were comparable regarding baseline characteristics (mean age ~60 years, similar ASA scores). In 2018, significant improvements were observed in: surgical wait times < 4 weeks (18% vs. 40.1%, p < 0.05), antibiotic prophylaxis compliance (0% vs. 95.8%, p < 10⁻⁵), surgical checklist observance (20% vs. 48%, p < 10⁻⁴), and pain management (31.1% vs. 100%, p < 10⁻⁵). The surgical site infection (SSI) rate decreased from 8.4% to 6.6%, though this difference was not statistically significant. Patient satisfaction rose from 49.5% to 82% (p < 0.05). Conclusion: The standardization of procedures significantly optimizes the clinical pathway and surgical safety. However, a larger sample size may be required to confirm a statistically significant reduction in SSI rates. Keywords: Digestive cancer, surgical safety, surgical site infection, clinical pathway, quality of care.
