Abstract: Acute ischemic stroke is the most common type of stroke accounting for 80% of the total number of strokes and the main cause of disability in adults. Intravenous Thrombolysis (IVT) is a proven effective therapy for patients suffering from ischemic stroke in the acute phase, it has a strong thrombolytic effect with a time window (3 to 4.5 hours). The American Heart Association (AHA) / American Stroke Association (ASA) recommends that the Door to needle time (DNT) is 60 minutes. The objective of our work is to describe our experience concerning the strategy of reducing intra-hospital time in order to improve the neurovascular sector during a period of 10 years at the EHU of ORAN. Method: This is a retrospective study of patients treated in the stroke unit of the November 1, 1954 hospital, including patients treated with Rt-PA in the Cerebrovascular Diseases Center of the EHU in Oran during the period: February 2015 to March 2024. Two interventions carried out from 2014 to 2019 (training of nursing staff) and from 2019 to 2024 (training of nursing staff). specialized neurologists). Results: 305 ischemic stroke patients were treated by IV thrombolysis including 130 women and 175 men. We note that the thrombolysis activity has experienced significant growth over the years, going from 6.6% in 2015 to a peak during the period 2021-2022. The DNT was measured for all 305 patients. The majority of patients, 68.9% (210 cases), benefited from thrombolysis within a period of less than 30 minutes after their arrival and the cross-tabulation of the average DNT per year and the evolution of the patients showed a very significant relationship. The NIHSS cross-tabulation of discharge of thrombolyzed patients and the average DNT showing a very significant relationship. Conclusion: A strategy for reducing and improving intra-hospital care must be developed in order to reduce the Door to Needle time which should not exceed 60 minutes according to international recommendations. This strategy must be based on the specialized and continuing training of the medical and paramedical teams of the neurovascular unit.
