Gastrointestinal hemangiomas are a benign, generally rare condition with a predilection for the small intestine. Due to their rarity, they can easily go undetected in clinical practice. Diagnosing small bowel hemangiomas can be complex, particularly because of their wide range of clinical features that mimic other conditions. While imaging is essential for diagnosis and treatment guidance, it can also lead to potential pitfalls that may result in confusion and misdiagnosis, potentially mistaking them for a tumor and thus complicating an accurate diagnosis. In this context, we present the case of a 58-year-old woman who presented with a small bowel mass that appeared following abdominal pain, suggestive of a tumor on imaging. We report this unusual case of difficult preoperative identification, where the diagnosis was ultimately established thanks to histopathological results obtained after segmental intestinal resection. The importance of this case is to keep this diagnosis in mind.
